Pathways to Wellness, LLC provides rehabilitative services to individuals diagnosed with auto-related and traumatic brain injuries (TBI). We offer long and short-term care, in a variety of settings. while administering the full support our patients need to improve their physical and mental health. We offer a comprehensive continuum of care, ranging from highly specialized programs tailored to each individual’s physical and/or functional limitations; to an increasingly less-structured setting, as goals are achieved.

The principals of Pathways To Wellness have provided quality services for more than 20 years. They have nurtured a company that manages outcome-based care plans administered by an experienced clinical team.

Our patients are cared for by a team of professionals who specialize in the care of trauma victims. The ultimate goals of rehabilitative care are: to restore most functional abilities (i.e. ability to move, use the bathroom, talk, eat); prevent secondary complications; and to stabilize medical and rehabilitation issues related to brain injuries and other injuries suffered, namely auto-related. Our team consists of physical therapists, occupational therapists and rehabilitation nurses.
